[Patient’s name, and rank:] Arius Syltius, SR1

[Time, and date of incident/operation:] October 12, 2186

[Materials used during incident/operation:] No materials used.

[The specific details of the operation, and/or incident.(Be as descriptive as possible, this will be logged.) :] Arius Syltius came into the medbay asking for a mental evaluation, I of course complied as he did not of a mental profile on record. After a few questions I concluded that Arius is suffering from a mental illness stemming from past experiences and I would prescribe him anti depressants.

[Severity of the situation, rate 1/100, and give a specific reasoning for the severity, (We don’t need 100/100 for a paper cut.):] 40/100

[Final thoughts?:] Arius Syltius displays characteristics of a psychopath as he fakes emotions and does not show any mercy or remorse on the battlefield, his lack of empathy is further stemmed from a traumatic event that happened early on his life, possibly due to the loss of a loved one and a rough childhood, further evaluation is required to determine a medical discharge.

Greetings Mrs. Zaaji, please answer the following inquiries as best as you can. I am not understanding much from this PCR.

What questions have you asked specifically of Syltius? Were they open-ended or close-ended questions? What specific details of your psychology assessment prompted you to prescribe him antidepressants, a class drug by which has serious side-effects if given to a clinically healthy patient. I ask this because if you provide an especially biased question, your patient WILL respond in a particular fashion.

Him "faking" emotions how? One can very easily say that they do not show mercy on the field, and I do not feel that a simple answer to an unknown question warrants him to be prescribed the drugs. Him lacking empathy for the enemy is common if he believes he is the "good guy;" by which is perfectly normal for anyone.

I would like to refer this patient over to my service for further evaluation. As of now, I am overriding your prescription to that patient's medications; he is to discontinue it ASAP. Please reply here or contact me directly when you are free. I would prefer it that you do not contact this patient until we have further talked of this matter.

Thanks!
