• Pt. states that he recently endured news of his sister dying, and has had both suicidal and murderous thoughts. Pt specified that he wants to kill members of the crew, elaborating the fact that he is constantly bullied by said members.
• Pt states that the bullying occurred in the past, stopped when he "socially withdrew" and returned when he started talking to another member of the crew.
• Pt admits that "Grayson" has physically assaulted him. Other crew members had shoved an object down his throat. Both have occurred weeks before.
• Pt noted that both Davey and the Captain knows of the harassment. Stated that no action has been taken against his harassers.
• Pt admits last attack was not memorable, and was in the far past.
• Pt clarifies that he wants to kill the entire crew, except for "Iz"
• Pt notes that previous medical officer called him crazy.

Actions taken:

• Pt assigned to myself for cognitive therapy.
• Pt considered a potential 17-99 and 51-50. He is not permitted to handle any weapons without an officer present.
• All psychological diagnosis are deemed null and void.
• Previous medical officer referred to commanding officers for disciplinary action for medical malpractice.
