Experience with staffing (ULX, other servers, etc.(Please Explain (What Servers etc)): I have been a staff on 2 Military RP servers, one was Melon RP server (no longer exists since the owner DDos'ed ever server that was Military RP) and the other was Darth RP (I was an admin for their Military RP and StarWars RP)

Have you ever been banned from any Revival servers? If so, why? Yes twice both by accident, they Unbanned me both times, they were aiming for someone else and accidently got me
How will your activity be on the server? Mostly trying to RP, if necessary stop people from breaking rules
Days, Hours Of Availability(If you go inactive, for 1 week without LOA or letting someone know anything, you can and will be removed from staff):
Sundays I will be available after 12 pm PST, Mondays- Wednesday, Friday- Saturday I should be available after 10 AM PST, Thursdays I should be available after 1pm PST, Other then that I will have days when I can't be on either for appointments or food shopping ( I will let you know at least 2 days in advance If necessary)

Name 5 Server Rules and explain the meaning of them -
No RDM: Do not randomly Kill someone without reason, whether that is RP or prior engagement (Such as SIM)
No ERP: Do not do anything that could me or is considered erotic, sexual, or something you wouldn't to in public
No FailRP: Anything that isn't RP such as Mic Spam, Meat-Gaming(Knowing something IC that you were told OOC) aka Minging, or just showing no intent to RP
No Spawn Camping: Staying in front or in someones spawn to then kill said person as they respawn (Only time you can is if they shoot you first, they shoot the corresponding person)
No Back Seat Moderating: Telling a Mod or Admin the rules while they are in a sit to stop, fix, or evaluate a situation, ie trying to tell the Mod/admin what to do
Name 5 IC Rules and explain the meaning of them -
Names Must Be Realistic: Have names corresponding with what you are such as Sangheili names as a Sangheili, and Human names as a Human, and nothing such as Ben Dover (Minging Names) and you can't us a codename unless the group/admin allows it
No Debilitating Physical or Mental illness, or Extreme Prejudices with new characters : You can't give you character either of these thing once you make them, they either have to be agreed on by admins or gained through experience.
No Baiting: Don't bait someone IC to try and get them PK'ed, such as provoking them by trying to make a scenario you made for them to get angry and attack
New Life Rule: Once you die your forget how you died, and if it's a PK, you forget everything in your past life (Unless it's a NPK then you know your training)
IC in SSR: When in the SSR you must remain in character unless stated by a sign in a specific area of the SSR

I, Dooby, have acknowledged that I have read the constitution for Revival Servers at http://revivalservers.com/constitution/ and fully understand how to abide by the rules
I,Dooby, Swear to uphold the server rules, and enforce them. And to stay within the limits and rights given to me by my assigned rank within Staff. And to keep a calm head and clear judgment when dealing with staff and or players, in order to ensure fairness to all who join the server.
And that if I Dooby, am found in violation of the rules I swore to uphold, and of this contract that I can and will be removed from Staff and forever barred from Re-Applying, along with any other Consequences that are deemed necessary.
And that I Dooby also will not under any Circumstance for whatever reason Retaliate against a player, Staff or this server, or the Community and any servers within it's domain for my own or someone else's feelings, or revenge.
I Dooby Acknowledge that this app may be denied or approved for a number of reasons. And that I will not retaliate if denied, and if accepted I will not flaunt it around in other's faces, that I will remain calm and cool-headed.
I Dooby swear that if I commit any acts, such as Violating Community Standard's Or Acts Such as Treason Towards the Server Or Community, I can and will be removed from Staff and Banned From Revival Servers, as it is a Just Punishment for Crimes committed.
